SUMMARY OF POSITION:
This role will develop, implement, and provide supervision of comprehensive drug therapy management. Provides education, mentoring and coaching for staff development. Participates in departmental and hospital programs directed at improving patient care standards. Oversees all clinical activities of the department and attends multidisciplinary committee meetings and rounds at the discretion of leadership.
MINIMUM QUALIFICATION(S):
Bachelor's degree in Pharmacy or Pharm D required.
Licensed to practice as an Pharmacist by the state of employment or license eligibility upon hire and licensure obtained within the first 90 days of hire.
Three (3) years of related experience required.
PREFERRED QUALIFICATION(S):
Board Certification in Pharmacology Specialty (BPS) preferred.
Pharmacy Graduate Year 1 (PGY-1) Residency experience preferred.
Pharmacist preceptor certification preferred.
Membership in at least one of the following professional organizations preferred: ASHP, ASCP, ACCP and/or specialty practice group(s).
