PRIMARY RESPONSIBILITIESActs as an operational liaison between select out-of-scope SSM Health teams; provide coordination across the care continuum with focus on case management.Manages internal and external stakeholder relationships to communicate progress, risks, issues, and mitigation to Care Transformation (CT). Leads to proactively drive resolution.Escalates issues, risks, disputes to the CT leadership, as appropriate.Maintains knowledge of latest best practices and industry insights related to case management for purposes of continued strategic planning and innovation. Support CT leadership in strategic planning, innovation, goal setting and other areas.Maintains knowledge of latest best practices and industry insights re
