Job Summary: Manages the activities of medical social workers and social work interns in an inpatient setting. Provides clinical and administrative support to social workers in assigned services or units in conducting initial screenings, psychosocial assessments and discharge planning. Provides support within the context of a multi-disciplinary team as it pertains to discharge planning, length of stay and prevention of avoidable readmissions. Support Social Workers with difficult discharge dispositions and Collaborates and maintains positive working relationships with physicians, interdisciplinary team members and post-acute care providers to ensure appropriate hospital length of stay and continuity of care. Responsibilities also involve evaluating and developing assigned social work staff and providing counseling when needed; Coordinating the functions of the social work staff; implementing the policies and procedures of the Social Work Department; planning and controlling the social work function on assigned units and services; developing and implementing departmental protocols, goals, policies and procedures in compliance with CMS, DOH and DNV regulations.
Responsibilities:
- Provides individual case supervision to Medical Social Work staff, on an as needed basis, in preparing psychosocial assessments, interventions and discharge planning.
- Coordinates formal and informal rounding with Medical Social Work staff and medical team (when available) to determine case disposition, patient progress, treatment goals, and possible new approaches for difficult cases.
- Conducts annual and probationary performance appraisals for assigned staff.
- Ensures compliance to hospital and regulatory standards by CMS, DOH, DNV and OMH as it pertains to Social Work and Discharge Planning.
- Conducts periodic chart reviews to ensure the timeliness and appropriateness of Social work Intervention(s) on assigned units.
- Oversees the coordination, interpretation, and integration of social work function with hospital administration and the leadership from other patient care departments and disciplines.
- Provides support to Social Work staff with difficult discharge dispositions and facilitates meetings with medical team, patients and families when appropriate.
- Coordinates and facilitates in-service training to social work staff and other disciplines on an assigned or as needed basis.
- Develops and maintains a list of Facilities/Agencies Contacts and referral resources needed in Discharge Planning and aftercare.
- Compiles and analyzes data to assist in planning, controlling and monitoring the activities of the Social Work Department.
- Supervises Clinic and Behavioral Social Workers, as needed".
Qualifications/Requirements:
Experience:
- 5 or more years of professional social work experience in a health care setting, required
Education:
- Master's degree in Social Work (M.S.W.) from a School of Social Work accredited by the Council on Social Work Education, required
Licenses / Certifications:
- Possession of a valid license and current registration, issued by the New York State Education Department, as a Licensed Clinical Social Worker (LCSW), required
